Evidence from primary care research also suggests that collaborative approaches to chronic disease management can positively influence clinical outcomes (Tandan et al., 2024).

Recent research supports the potential role of telemedicine in chronic disease management. A systematic review and meta-analysis examining patients with hypertension and type 2 diabetes in primary care found that certain telemedicine approaches—particularly telemonitoring combined with teleconsultations—were associated with improvements in important cardiovascular risk factors, including systolic blood pressure and, in some circumstances, blood glucose control (Mihevc et al., 2025).
A separate systematic review and meta-analysis also found that telehealth chronic disease management systems may improve several aspects of quality of life among people living with chronic illnesses (Xiao & Han, 2023).
